A death certificate is required to settle bank accounts, insurance, property, and pensions. This free template requests it from the municipal authority with the dignity and completeness this difficult paperwork deserves.

When and how to use this letter
Register the death promptly with the municipal authority. The hospital or attending doctor issues a medical certificate of cause of death; for deaths at home, a registered medical practitioner's certificate is needed. Carry the deceased's CNIC or ID proof along with your own.
Apply for multiple certified copies at the same time, at least four or five. Banks, insurance companies, pension offices, property registrars, and courts each demand an original, and returning for more copies later means repeating the whole queue.
The death certificate unlocks everything that follows: closing bank accounts, claiming insurance, transferring property, stopping pensions, and settling utility connections. List every institution that will need it and work through them systematically with a copy of the certificate and your relationship proof.
A non-obvious tip: get the certificate before you start closing accounts or transferring property. Some banks freeze accounts on learning of a death and will only release funds against the certificate plus a succession certificate or court order, so having the death certificate ready avoids the account being stuck in limbo.
Check the name spellings and date on the certificate immediately. Errors here cascade into property and banking documents, and corrections require affidavits or court orders. If the deceased's name differs across documents, carry the supporting IDs that link them.
